Payment Options
Prepayment Savings: Prepay 48 hours in advance of your appointment by Check or Cash and receive a 10% discount. Prepay 48 hours in advance of your appointment by Credit or Debit Card and receive a 6% discount. Insurance reimbursements will be directed to the patient's mailing address.
Insurance: The estimated patient portion, any deductibles or co-payments must be paid when services are rendered.Any remaining balance after insurance payments must be paid within 30 days of the statement date.
